      目的探讨超声引导下微波消融治疗甲状腺良性结节的安全性及近期有效性。方法选取 2017年 7月至 2019年 6月在安徽中医药大学第一附属医院就诊的 34例甲状腺良性结节病人,排除禁忌证均接受超声引导下微波消融治疗,观察记录围手术期并发症及不良反应,术后 1、3、6个月随访甲状腺结节的体积缩小率( volume-reduction ratio,VRR)。结果 34例病人均无严重的并发症。本研究 43枚结节均顺利完成微波消融治疗,一次性消融率为 100%。实性结节术前体积( 12.13±16.35)mL,术后 1个月、 3个月、 6个月体积为( 10.85±15.52)mL、(7.56±12.10)mL、(4.96±9.20)mL,VRR为(14.29±5.51)%、(49.65±12.12)%、

      Objective To investigate the safety and short-term effectiveness of ultrasound-guided microwave ablation in the treat-ment of benign thyroid nodules.Methods Thirty-four patients with benign thyroid nodules who were treated in the First Affiliated Hos-pital of Anhui Traditional Chinese Medical University from July 2017 to June 2019 were selected. After exclusion of contraindications,all patients received ultrasound-guided microwave ablation. Perioperative complications and adverse reactions were observed and re-corded. Volume-reduction ratio (VRR) of thyroid nodules was followed up 1, 3 and 6 months after operation.Results There were no se-rious complications in 34 patients. In this study, 43 nodules were successfully treated by microwave ablation, and the ablation rate was100%. The volume of solid nodules before operation was (12.13 ± 16.35) mL, and the volume at 1 month, 3 months and 6 months afteroperation were (10.85 ± 15.52) mL, (7.56 ± 12.10) mL and (4.96 ± 9.20) mL, respectively. The volume reduction rates were (14.29 ±5.51)%, (49.65 ± 12.12)% and (77.18 ± 15.38)%, respectively. The volume of cystic and solid nodules before operation was (11.76 ±14.43) mL. The volume at 1 month, 3 months and 6 months after operation was (9.01 ± 11.02) mL, (5.14 ± 6.82) mL and (2.04 ± 2.78)mL, respectively. The volume reduction rates were (22.88 ± 8.73)%, (56.85 ± 8.74)% and (85.31 ± 6.62)%, respectively. After micro-wave ablation of solid and cystic solid nodules, the volume of nodules decreased gradually at 1, 3 and 6 months, and the VRR of nod-ules at 1, 3 and 6 months after microwave ablation was statistically significant (P < 0.05). Compared with VRR of the two groups at dif-ferent time after microwave ablation, the cystic solid nodule was significantly better than the solid nodule at 1 and 3 months postopera-tively, and the difference between the two groups was statistically significant (P < 0.05), but there was no significant difference between the two groups at 6 months (P = 0.102).Conclusion Ultrasound guided microwave ablation for benign thyroid nodules is characterizedby less complications and significant curative effect. It is a safe, effective and worthy of clinical promotion.
