      目的探究桥本甲状腺炎( HT)病儿血清肿瘤坏死因子受体相关因子 6(TRAF6)和神经生长因子受体( NGFR)水平与免疫水平及甲状腺功能的相关性。方法选取 2021年 3月至 2022年 3月在承德市妇幼保健院就诊的 HT病儿 104例作为 HT组,根据甲状腺功能分为 HT亢进组 30例, HT正常组 38例, HT减退组 36例。采用随机数字表法选取同期在儿科门诊体检健康的儿童 95例作为对照组。采用酶联免疫吸附测定( ELISA)检测血清辅助型 T细胞 1(Th1)因子中的白细胞介素 2(IL-2)、 γ-干扰素(IFN-γ)、辅助型 T细胞 2(Th2)因子中的白细胞介素 4(IL-4)以及 TRAF6、NGFR水平;采用 Pearson法分析血清 TRAF6、NGFR表达以及二者与甲状腺功能指标、炎症因子的相关性。结果 HT正常组、 HT减退组和 HT亢进组 TRAF6的表达水平显著高于对照组[(25.12±3.36)μg/L、(28.16±3.48)μg/L、(22.65±3.27)μg/L比(19.25±2.42)μg/L]HT正常组和 HT减退组表达水平显著高于 HT亢进组, HT减退组显著高于 HT正常组( P<0.05); HT正常组、 HT减退组和 HT亢进,组 NGFR的表达水平显著低于对照组[( 27.36±3.48)μg/L、(25.02±3.42)μg/L、(29.74±3.56)μg/L比( 32.04±3.71)μg/L],HT正常组和 HT减退组表达水平显著低于 HT亢进组, HT减退组显著低于 HT正常组( P<0.05); Pearson相关性分析结果显示, HT病人血清中 TRAF6和 NGFR表达水平呈负相关(P<0.05);与对照组相比, HT组血清中 IL-2、IFN-γ、TPOAb、TGAb、游离三碘甲状原氨酸、血清三碘甲状原氨酸、甲状腺素、甲状腺球蛋白水平显著升高, IL-4、游离甲状腺素水平显著降低( P<0.05); Pearson法分析显示, TRAF6水平与 IL-2、IFN-γ、 TPOAb、TGAb、游离三碘甲状原氨酸、血清三碘甲状原氨酸、甲状腺素、甲状腺球蛋白水平呈正相关,与 IL-4、游离甲状腺素水平呈负相关( P<0.05),NGFR水平与 IL-4、游离甲状腺素水平呈正相关,与 IL-2、IFN-γ、TPOAb、TGAb、游离三碘甲状原氨酸、血清三碘甲状原氨酸、甲状腺素、甲状腺球蛋白水平呈负相关( P<0.05)。结论 HT病人血清 TRAF6表达水平异常升高, NGFR水平异常降低,均与免疫水平及甲状腺功能相关。

      Objective To investigate the correlation between serum tumor necrosis factor receptor-related factor 6 (TRAF6) andnerve growth factor receptor (NGFR) levels and immune levels and thyroid function in children with Hashimoto's thyroiditis (HT).Methods The prospective study included 104 children with HT who were admitted to Chengde Maternal and Child Health Hospitalfrom March 2021 to March 2022 as the HT group. According to the thyroid function, they were divided into 30 cases in HT hyperactive group, 38 cases in HT normal group and 36 cases in HT decompensated group. Ninety-five healthy children who were physicallyexamined in the pediatric outpatient clinic were randomly selected as the control group. The levels of interleukin-2 (IL-2) and γ-interferon (IFN-γ) in serum helper T-cell 1 (Th1) factor, interleukin 4 (IL-4) in helper T-cell 2 (Th2) factor, and TRAF6 and NGFR were measured by enzyme-linked immunosorbent assay (ELISA); Pearson method was used to analyze serum TRAF6 and NGFR expressionand the correlation between the two and thyroid function indexes and inflammatory factors.Results The expression level of TRAF6in normal HT group, hypoactive HT group and hyperactive HT group was significantly higher than that in control group [(25.12±3.36)μg/L,(28.16±3.48)μg/L,(22.65±3.27) μg/L vs. (19.25±2.42) μg/L], the expression level in normal HT group and hypoactive HT groupwas significantly higher than that in hyperactive HT group, and the expression level in hypoactive HT group was significantly higherthan that in normal HT group (P<0.05). The expression level of NGFR in normal HT group, hypoactive HT group and hyperactive HTgroup was significantly lower than that in control group [(27.36±3.48) μg/L,(25.02±3.42) μg/L,(29.74±3.56) μg/L vs. (32.04±3.71)μg/L], the expression level in normal HT group and hypoactive HT group was significantly lower than that in hyperactive HT group,and the expression level in hypoactive HT group was significantly lower than that in normal HT group (P<0.05). Pearson correlationanalysis showed that there was a negative correlation between TRAF6 and NGFR expression levels in serum of HT patients (P < 0.05). Compared with the control group, the serum levels of IL-2, IFN-γ, TPOAb, TGAb, free triiodothyronine, serum triiodothyronine, thyroxine and thyroglobulin in the HT group were higher, and the levels of IL-4 and free thyroxine were lower (P<0.05); Pear-son's method analysis showed that TRAF6 level was positively correlated with IL-2, IFN-γ, TPOAb, TGAb, free triiodothyronine, serum triiodothyronine, thyroxine, and thyroglobulin levels, and negatively correlated with IL-4 and free thyroxine levels (P<0.05). NGFR level was positively correlated with IL-4 and free thyroxine levels, and negatively correlated with IL-2, IFN-γ, TPOAb, TGAb, free triiodothyronine, serum triiodothyronine, thyroxine, and thyroglobulin levels (P<0.05). Conclusion Serum TRAF6 expressionlevel is abnormally elevated in HT patients and NGFR level is abnormally reduced, both of which are associated with immune levelsand thyroid function.
