      目的探讨环状 RNA原肌球调节蛋白 3(circTMOD3)在细菌性肺炎病儿血清中的水平及其与病情严重程度和预后的相关性。方法前瞻性研究。选取 2021年 1月至 2022年 6月在邯郸市第一医院接受治疗的 118例细菌性肺炎病儿进行研究,据肺炎病儿的病情严重程度分为轻症组( n=72)和重症组( n=46)根据肺炎病儿的预后情况分为预后良好组( n=95)和预后不根良组( n=23);另选取同期体检健康儿童 118例作为健康组。采用实时,荧光定量逆转录聚合酶链反应( qRT-PCR)检测各组血清 circTMOD3水平;比较各组血清 C反应蛋白( CRP)、降钙素原( PCT)水平及临床肺部感染评分( CPIS);受试者操作特征曲线(ROC曲线)分析血清 circTMOD3、CRP、PCT水平对肺炎病儿预后的评估价值。结果重症组病儿血清 circTMOD3(2.65±0.91比 1.97±0.65、1.02±0.23)、 CRP、PCT水平显著高于轻症组和健康组( P<0.05)重症组病儿 CPIS显著高于轻症组( P<0.05)轻症组病儿血清 circTMOD3、CRP、PCT水平显著高于健康组( P<0.05)。与预后良好,组相比,预后不良组病儿血清 circTMOD3(3.,54±1.28比 1.92±0.63)、 CRP、PCT水平以及 CPIS显著升高( P<0.05)。 ROC曲线显示,血清 circTMOD3、CRP、PCT联合检测预测细菌性肺炎病儿预后的曲线下面积( AUC)为 0.97,灵敏度为 95.65%、特异度为 91.58%,优于三者各自单独预测(均 P<0.05)。结论 circTMOD3在细菌性肺炎病儿血清中高表达,且与病儿的病情严重程度和预后密切相关,其对病人预后具有较高的评估效能,且与 CRP、PCT联合检测评估效能更高。

      Objective To investigate the expression level of circular RNA tropomodulin 3 (circTMOD3) in the serum of children withbacterial pneumonia and its correlation with the severity and prognosis of the disease.Methods A prospective study. Between January2021 and June 2022, 118 children with bacterial pneumonia treated at Handan First Hospital were included. They were assigned tomild group (n=72) and severe group (n=46) according to disease severity, and to good prognosis group (n=95) and poor prognosis group (n=23) according to their prognosis; another 118 healthy children who underwent the same physical examination and were found to behealthy were selected as healthy group. The level of circTMOD3 in serum of each group was detected by real-time fluorescence quanti. tative reverse transcription polymerase chain reaction (qRT-PCR); Compare the levels of serum C-reactive protein (CRP), procalcitonin(PCT), and the clinical pulmonary infection score (CPIS) in each group; Receiver operating characteristic curve (ROC curve) was ap.plied to analyze the prognostic value of serum circTMOD3, CRP, and PCT level in children with pneumonia.Results The serum lev. els of circTMOD3 (2.65±0.91 vs. 1.97±0.65, 1.02±0.23), CRP and PCT in severe group were obviously higher than those in mild group and healthy group (P<0.05), the CPIS score of children in severe group was obviously higher than that in mild group (P<0.05), the se. rum levels of circTMOD3, CRP and PCT in the mild group were obviously higher than those in the healthy group (P<0.05). Compared with the good prognosis group, the serum levels of circTMOD3 (3.54±1.28 vs. 1.92±0.63), CRP, PCT and CPIS in the poor prognosis group were obviously higher (P<0.05). ROC curve showed that the area under the curve (AUC) of the combined detection of serumcircTMOD3, CRP, and PCT in predicting the prognosis of children with bacterial pneumonia was 0.97, the sensitivity was 95.65%, andthe specificity was 91.58%, which was better than that of the independent detection (all P<0.05).Conclusions CircTMOD3 is highlyexpressed in the serum of children with bacterial pneumonia, and is closely related to children with pneumonia severity and prognosisof patients. It has a high assessment efficacy for the prognosis of patients, and the combined detection of CRP and PCT has an evenhigher assessment efficacy.
