| 目的探索成人先天性胆管扩张症 Ⅳ-A型治疗经验及文献复习。方法总结天门市第一人民医院 2024年 11月收治的 1例成人先天性胆管扩张症 Ⅳ-A型病人的治疗方案。结果 39岁男性病人主诉呕吐、纳差、尿黄 5d。初步诊断为肝功能不全，入院第 8天行腹腔镜下胆管囊肿切除术及腹腔镜下胆管空肠吻合术。术后 10 d复查肝功能明显好转，症状好转出院。术后 3月余复查肝功能提示:总胆红素 26.0 μmol/L，直接胆红素 6.3 μmol/L。结论先天性胆管扩张症在成人中症状多不典型，且常伴有肝功能损伤，极易误诊，早期手术治疗、合理重建胆肠通路是针对该病唯一有效的治疗手段。 |

| Objective To share the treatment experience of adult congenital bile duct dilatation type Ⅳ-A and review the literature. Methods The treatment plan of an adult patient with congenital bile duct dilatation type Ⅳ -A admitted to Tianmen First People's Hospital in November 2024 was summarized.Results A 39-year-old male patient presented with vomiting, poor appetite, and yellowurine for 5 days. He was initially diagnosed with liver insufficiency. On the 8th day of hospitalization, laparoscopic choledochal cyst ex.cision and laparoscopic cholangiojejunostomy were performed. Ten days after surgery, liver function had improved significantly, and thesymptoms resolved, allowing discharge. At more than 3 months postoperatively, liver function tests showed a total bilirubin level of 26.0μmol/L and a direct bilirubin level of 6.3 μmol/L.Conclusions Congenital bile duct dilatation in adults often presents with atypicalsymptoms and is frequently accompanied by liver function impairment, making it easily misdiagnosed. Early surgical treatment withreasonable biliary-enteric reconstruction is the only effective treatment modality for this disease. |
