目的检测脑出血病人血清中微 RNA(miRNA)-183-5p和 miRNA-144-3p表达水平,分析两者与脑出血量及认知功能的相关性。方法回顾性分析 2020年 8月至 2021年 8月荆州市第一人民医院收治的符合纳入、排除标准的 89例脑出血病人的临床资料(观察组),另收集体检健康者 89例为对照组。根据头颅影像学检查结果,将观察组分为少量出血组( <15 mL)31例,中量出血组( 15~30 mL)36例和大量出血组( >30 mL)22例。采用实时荧光定量聚合酶链式反应( qRT-PCR)检测血清 miRNA183-5p和 miRNA-144-3p表达水平, Pearson相关分析脑出血量与血清 miRNA-183-5p、miRNA-144-3p表达水平的相关性,根据是否出现认知功能障碍,将观察组病人分为预后良好组 57例和预后不良组 32例;受试者操作特征曲线( ROC曲线)分析血清 miRNA-183-5p和 miRNA-144-3p水平对脑出血病人预后的评估价值。结果观察组血清 miRNA-183-5p表达水平 0.67±0.16低于对照组 1.01±0.24,血清 miRNA-144-3p的表达水平 3.53±0.73高于对照组 1.05±0.21(P<0.05)。大、中、少出血组血清 miRNA183-5p水平为 0.49±0.11、0.69±0.11、0.78±0.21,依次显著降低( P<0.05):大量出血组血清 miRNA-144-3p水平 3.86±0.75与中量出血组水平 3.58±0.68显著高于少量出血组水平 3.17±0.65(P<0.05)。脑出血量与血清 miRNA-183-5p表达呈负相关( r=?0.51, P<0.05)、与 miRNA-144-3p表达呈正相关( r=0.54,P<0.05)。预后不良组血清 miRNA-183-5p水平 0.50±0.12低于预后良好组0.77±0.18,血清 miRNA-144-3p水平 3.75±0.62高于预后良好组 3.41±0.34(P<0.05)。血清 miRNA-183-5p、miRNA-144-3p单独及联合评估病人预后的 AUC分别是 0.94、0.79和 0.96,二者联合检测的 AUC、灵敏度高于各自单独检测。结论脑出血病人血清 miRNA-183-5p水平降低, miRNA-144-3p水平升高,与脑出血量相关性较强,二者联合可评估病人认知功能障碍的发生。 |

Objective To detect the expression levels of miRNA-183-5p and miRNA-144-3p in serum of patients with cerebral hemorrhage, and to analyze the correlation between the two with the amount of cerebral hemorrhage and cognitive function.Methods The clinical data of 89 patients with intracerebral hemorrhage who met the inclusion and exclusion criteria admitted to Jingzhou First People's Hospital from August 2020 to August 2021 were retrospectively analyzed (observation group). Another 89 healthy subjects' clinical data who underwent physical examination were collected as the control group at the same time. According to the results of skull imaging, the observation group was divided into different subgroups: 31 cases in the small amount of bleeding group (<15 mL), 36 cases inthe medium amount of bleeding group (15-30 mL) and 22 cases in the large amount of bleeding group (>30 mL). The expression levels of miRNA-183-5p and miRNA-144-3p in serum of patients were detected by quantitative real-time polymerase chain reaction (qRT-PCR), Pearson correlation was applied to analyze the correlation between the amount of cerebral hemorrhage and the expression levelsof miRNA-183-5p and miRNA-144-3p in serum, the patients were grouped into a good prognosis group (n=57) and a poor prognosis group (n=32) according to whether there was cognitive impairment or not; receiver operating characteristic (ROC) curve was applied toanalyze the appraisal value of serum miRNA-183-5p and miRNA-144-3p levels in patients with intracerebral hemorrhage. Results The serum miRNA-183-5p expression level in the observation group 0.67±0.16 was lower than that in the control group 1.01±0.24, and the serum miRNA-144-3p expression level 3.53±0.73 was higher than that in the control group 1.05±0.21 (P<0.05). The level of serum miRNA-183-5p in the groups with different bleeding volume was that in the group with large bleeding volume, medium bleeding volume and small bleeding volume were 0.49±0.11, 0.69±0.11, 0.78±0.21 (P<0.05). The level of serum miRNA-144-3p in the groups withlarge bleeding volume 3.86±0.75 and medium bleeding volume 3.58±0.68 was significantly higher than that in the group with smallbleeding volume 3.17±0.65 (P<0.05). The blood loss in patients with cerebral hemorrhage was negatively correlated with the expression of serum miRNA-183-5p (r=?0.51, P<0.05), and positively correlated with the expression of miRNA-144-3p (r=0.54, P<0.05). The level of serum miRNA-183-5p of patients with cerebral hemorrhage in the poor prognosis group 0.50±0.12 was lower than that in the goodprognosis group 0.77±0.18, the level of serum miRNA-144-3p in the poor prognosis group 3.75±0.62 was higher than that in the good prognosis group 3.41±0.34 (P<0.05). The AUC of serum miRNA-183-5p and miRNA-144-3p alone and in combination to evaluate theprognosis of patients was 0.94, 0.79 and 0.96. The AUC and sensitivity of the combined detection were higher than those of the independent detection.Conclusions Serum miRNA-183-5p level is decreased and miRNA-144-3p level is increased in patients with cerebralhemorrhage, which are strongly correlated with the amount of cerebral hemorrhage. The combination of the two can evaluate the occurrence of cognitive dysfunction in patients. |
