| 摘要:目的：通过回顾性分析肛周坏死性筋膜炎患者的临床资料，探讨肛周坏死性筋膜炎预后的影响因素，对临床治疗提供依据。方法：对南京中医药大学附属南京中医院2014年~2022年间诊断为肛周坏死性筋膜炎的136例患者进行分析，比较术后死亡患者和存活患者的临床资料，并将单因素指标代入多因素Logistic回归模型进行多因素分析。结果：在136例PNF患者中，112例患者成功治愈，24例患者死亡，病死率达17.6%。单因素分析结果显示，两组患者C-反应蛋白、白细胞计数、血糖、肌酐、尿素氮的差异具有统计学意义（P＜0.05）。进一步Logistic回归模型结果分析显示，C-反应蛋白、白细胞计数、血糖、肌酐为肛周坏死性筋膜炎诊治预后的独立影响因素（P＜0.05），且联合预测具有较高的准确性。结论：C-反应蛋白、白细胞计数、血糖、肌酐联合对肛周坏死性筋膜炎的预后有较高的准确性。 |

| Abstract:Objective: By retrospectively analyzing the clinical data of patients with perianal necrotizing fasciitis, we explored the factors influencing the prognosis of perianal necrotizing fasciitis and provided a basis for clinical treatment. Methods: 136 patients diagnosed with perianal necrotizing fasciitis in Nanjing Hospital of Traditional Chinese Medicine affiliated to Nanjing University of Traditional Chinese Medicine between 2014 and 2022 were analyzed to compare the clinical data of patients who died after surgery and those who survived, and substituting the single-factor indexes into the multifactorial logistic regression model for multifactorial analysis. Results: Among the 136 patients with PNF, 112 patients were successfully cured and 24 patients died, with a morbidity and mortality rate of 17.6%. The results of univariate analysis showed that the differences in C-reactive protein, white blood cell count, blood glucose, creatinine, and urea nitrogen between the two groups were statistically significant (P < 0.05). Further logistic regression model results analysis showed that C-reactive protein, leukocyte count, blood glucose, and creatinine were independent influencing factors for the prognosis of the diagnosis and treatment of perianal necrotizing fasciitis (P < 0.05), and the joint prediction had a high accuracy. Conclusion: The combination of C-reactive protein, leukocyte count, blood glucose, and creatinine had high accuracy in the prognosis of perianal necrotizing fasciitis. |
